The Medically Supervised Chemical Dependency Outpatient Treatment Program at CREATE provides recovery services to men and women.
These services include medical and psychological services, vocational counseling, job placement and basic computer skills, and help for specific groups of people including those with specific issues.
These additional services may involve parole status, mentally ill chemical abusers, anger management, domestic violence and those in need of lifestyle directions and vocational assistance.
The Co-Occurring Mental Illness and Substance Abuse Program made its debut in 2008. The program goal is to provide much needed services to consumers presenting issues with substance abuse and/or dependence, as well as a psychiatric condition/diagnosed mental disorder.
In addition to medical supervision, we provide services offered by our staff Mental Health Counselors and psychiatrist. Our psychiatrist is available to provide psychiatric screenings/assessments, ongoing outpatient psychiatric care, and psychiatric medication, as needed. Our Mental Health Counselors provide psychological evaluations, reports for various agencies (ACS, HRA, SSI/SSD, etc.), ongoing individual psychotherapy and group therapy.
Continental breakfasts and nutritious lunches are served daily.
All services at CREATE are provided by trained LMHC's, LCSW's, RN's, Phsychiatrists, Medical Doctors, CASAC's and CRC's.
Our Evidenced based practices include Motivational Interviewing, Cognitive Behavioral, Seeking Safety, Thinking for a Change and Contingency Managment.
The three phases of treatment are based on the trans-theoretical model.
Domestic Violence, Anger Management and Families in Recovery Groups are offered.
Multiple tracks of treatment include Intensive Outpatient Treatment, a Youth Track, a Mental Wellness Traing and program flexibility designed for employed participants and participants with child care needs.
Referrals to in-house Health Homes case management.
Opiate and alcohol medical managemnet to assist with the recovery process (such as Suboxone, Vivitrol, Campral and Naltrexone).
CREATE has in-hosue Psychiatric Assessment, Medication Management and Psychotherapy.
We also offer rapid HIV, Hepatitis C and STD testing.
Group curriculum includes anger management, Women's/Men's parenting, process, weekend in recovery and employment in recovery.
A Vocational Counselor is available for assessment and referral to Access-VR and community-based job training programs.
Biligual services in Spanish are available.
Daily Lunch and Transportation are provided.
Entitlement Enrollment is provided every week.
