CREATE, Inc. provides a full array of services to people in need. We have several locations within Harlem, and our services include: treatment and recovery services for alcohol- and chemically-dependent men and women; specialized services for those who have co-occurring mental health and chemical abuse issues; transitional living for homeless young adults; permanent housing and support services for single persons in recovery; vocational counseling, job preparation and placement; and emergency food distribution for the hungry.

Our programs undergo rigorous oversight by city and state agencies to make sure we are in compliance with all licensing and safety regulations. We participate in the Health Homes network, Medicaid, and other comprehensive organizations providing social services and medical care.

For many clients needing care coordination, New York Columbia Presbyterian (NYP) coordinates many services for Health Home entities designated by the New York State Department of Health. CREATE works in partnership with medical and behavioral health providers throughout New York City and Long Island to coordinate services that promote the well being of Medicaid recipients enrolled in the Health Home program. Care Coordination teams use a person-centered approach to provide assessment, service planning and referral for individuals living with medical conditions, mental illness and substance use disorders.

NYP’s Care Coordination program enhances outcomes by ensuring that clients are linked to appropriate treatment settings. These linkages serve to reduce emergency room visits, unnecessary hospital admissions and readmissions, and referrals to long-term care facilities. The goal of Health Home care coordination is to reduce Medicaid expenditures by improving the overall health of participants.

As a downstream partner for New York Columbia Presbyterian, we seek to help individuals with housing and vocational needs, along with phsychological and medical treatment.

CREATE is a not-for-profit organization that provides 24-hour-a-day Transitional Shelter, In-Patient and Out-Patient Rehab and Counseling services. We are here to help you.
