We provide a host of substance abuse, mental health services and housing services to help serve the Harlem Community.
CREATE, Inc. is a 501(c)3 not-for-profit organization that has been serving the Harlem Community since 1973 for Outpatient and Inpatient substance abuse treatment, as well as providing Young Adult Transitional Housing and Permanent Supportive Housing. There has been a lot happening in the CREATE family lately and we will be sure to share all of the details with you shortly.
In the meantime, we share some sad news: Father Benedict Taylor, friend and supporter of CREATE for decades, recently passed away. We invite you to view this short video, taken a few months before his death, so that you may get to know our beloved friend. We are very grateful for the love and support he shared with us for nearly 50 years.
The Medically Supervised Outpatient Program identifies each participant's needs, and is based on a holistic approach that follows a medical model to evaluate, assess and develop a treatment plan, agreed upon by the participant, primary counselor and Multidisciplinary professionals. As a clinical team, we focus on each participant's level of care, which includes many components: medical and mental health, educational and vocational status, and suitable housing.
CREATE, Inc. also operates a Food Pantry on Wednesdays that serves our hungry neighbors.Read More
Case Management services are available to all participants who are referred to CREATE, Young Adult or other residential programs.
Case Management involves a coordinated treatment plan for those with substance abuse, medical, social or housing issues led by our NADAP representative.Read More
