Permanent Supportive Housing Program provides housing for individuals and or families who are undomiciled and in re-entry from incarceration or completing transitional treatment after incarceration and have a substance-abuse and/or mental health diagnosis. Through this initiative CREATE provides previously incarcerated individuals the opportunity to live in a supervised environment that supports independent living, provides peer support, facilitates access to agency and community resources, wraparound and evidence-based practices that will build the foundation to a successful community reentry.
